    Laboratory Safety Rules For The Students

    To prevent something wrong in laboratory, the student must obey and follow the instructions below. These instructions are available in every laboratory.
    1. Conduct yourself in a responsible manner at all times in the laboratory.
    2. Follow all written and verbal instructions carefully.If you do not understand a direction or part of a procedure, ASK YOUR TEACHER BEFORE PROCEEDING WITH THE ACTIVITY.
    3.When first entering a science room, do not touch any equipment, chemicals or other materials in the laboratory area until you are instructed to do so.
    4. Do not eat food, drink beverages, or chew gum in the laboratory. Do not use laboratory glassware as containers for food or beverages.
    5. Any time chemicals, heat or glassware are used, students will wear safety goggles. NO EXCEPTIONS TO THIS RULE!
    6. Keep hands away from face, eyes, mouth, and body while using chemicals or lab equipment.
    Wash your hands with soap and water after performing all experiments.

    General Laboratory Safety

    MEDICAL PROGRAM
    A. Medical surveillance, including medical consultation and follow-up, shall be provided under the
    following circumstances:
    1. Where exposure monitoring reveals exposure over the action level (or PEL if there is no
    action level) for an OSHA regulated substance which has medical surveillance
    requirements.
    2. Wheneve r a laborato ry emp loyee deve lops sign s or sym ptom s that m ay be ass ociated
    with a hazardous chemical to which the employee may have been exposed to in the
    labor atory.
    3. Whenever a spill, leak, or explosion results in the likelihood of a hazardous exposure.
    4. For any employees using respiratory protection.
    B. All medical examinations sha ll be provided by a licensed physician or under direct supervision of a
    licensed physician, at no cost to the employee, without loss of pay, and at a reasonable time and
    place. Medical monitoring programs should be coordinated with the Occupational Health Unit at
    the University Health Center (301-314-8199).
    C. First aid kits are to be supplied and maintained by the LS/PI. Additional non-emergency medical
    assistance, if required, is available at the University Health Center. Emergency medical
    assistance is available by calling 911. Emergency phone numbers shall be posted in each lab
    (UM Em ergency Respons e Guide).
    D. Where me dica l cons ultatio ns or exam inatio ns ar e pro vided , the exam ining p hysic ian sh all be
    provided with the following information:
    1. The identity of the hazardous chemical(s) to which the employees may have been
    exposed.
    2. A description of the conditions under which the expos ure occurred including quantitative
    exposure data if available.
    3. A de scrip tion o f the s igns and s ymptom s of e xposure that th e em ployee is exp erien cing , if
    any.
    If the in jured labor atory employee is trans porte d by am bulan ce an d is un able t o pro vide th is
    information a t the tim e of treatm ent, a cow orke r sho uld ac com pany the employee t o the hosp ital.
    E. Medical examinations or consultations provided to employees shall be maintained at the
    University Health Center and available per the requirements of 1910.20 "Access to Employee
    Exposure and Medical Records." A written opinion from the examining physician shall be
    provided to the laboratory supervisor or Chemical Hygiene Officer. It shall include:
    1. Recommendations for further medical follow up.
    2. Results of the examination and associated tests.
    3. Any medical condition that places the employee at increased risk due to exposure to a
    hazardous substance found in the workplace.
    4. A statement that the employee has been informed of the results of the examination or
    consultation.
    F. Accide nts
    1. Injuries, wh ich occu r in the labora tory mu st be immediately treated.
    2. Injuries requiring first aid may be treated using the first aid kit provided/maintained by the
    LS/PI in each lab.
    3. Emerge ncy transport (ambulance) sh all be contacted to respond to injuries requiring more
    extens ive treatm ent.
    4. All injuries shall be investigated by the employee's immediate supervisor (or designate)
    and reported to the UM C hemical Hygiene Officer.
    5. Lab incidents (without injury) should also be reported and reviewed with the Chemical
    Hygiene Officer.

    I. TUJUAN
    Memfasilitasi dan membantu siswa dalam rangka pengembangan serta penambahan kemampuan siswa dalam berbahasa Inggris.
    II. RUANG LINGKUP
    Semua siswa-siswi SMA Muhammadiyah 04 Andong Boyolali yang mengikuti mata pelajaran bahasa Inggris di sekolah.
    III. ACUAN
    Misi SMA Muhammadiyah 04 Andong Boyolali adalah “ Mampu menciptakan siswa-siswi yang cakap dan terampil guna menyongsong perubahan zaman serta menjadi tauladan bagi sekolah-sekolah yang lain”.
    IV. SARANA
    30 unit peralatan laboratorium bahasa, 1 unit master control, perangkat audio visual, dan alat tulis.
    V. PROSEDUR
    a. Guru mata pelajaran mengajukan surat ijin penggunaan Lab Bahasa yang ditujukan kepada Kepala Sekolah.
    b. Kepala Sekolah memerintahkan kepada kepala Lab Bahasa untuk mempersiapkan segala sesuatu yang berhubungan dengan proses kegiatan tersebut termasuk segala bentuk administrasi maupun rencana anggaran pelaksanaan kegiatan tersebut.
    c. Kepala Lab Bahasa dan pengelolanya mempersiapkan sarana prasarana, pengaturan jadwal serta berkoordinasi dengan Guru Mata Pelajaran berkenaan dengan hal-hal apa saja yang berkaitan dengan kegiatan tersebut.

    SOP Laboratory Language for the student
    Because of the sensitivity of all the tools inside the laboratory, each student should follow the instruction to use the language laboratory. This instruction should be followed by the students when using the laboratory. Read the instruction below.
    1. Put your bag on the bag place in front if the room.
    2. Find the booth number that appropriate with your attendance number.
    3. Start to use the tools (LCD, headphone, etc.) carefully after the teacher's instruction.

    Purpose
    Facilitate and assist non-educational institutions outside the Polytechnic environment
    Yogyakarta Department of health in development as well as the addition of traffic in the language.

    Scope
    All non-educational institutions outside the Department of Health Polytechnic environment Yogyakarta.

    Reference
    Yogyakarta Department of Health Polytechnic mission “to develop itself into an information center and referral to Government Health Institutions and private education in Indonesia as well as for the general public”.

    Means
    40 units of language laboratory equipment, a master control unit and audiovisual equipment and stationery.

    Procedure

    Step Installing Windows XP with USB :
    1. Plug in a USB flash drive into any USB port. Remember his drive position. Is the F:, G:, H:, and so on.

    Fires can happen anywhere, as well as in the multimedia laboratory. This could occur because of the short-circuit or electric shock. And there are some step to save yourself or student who use the lab. Please attention and follow the instruction below.

    1. Stay calm in the face of fire.

    2. If a small fire and can still be solved, immediately put out with extinguishers available as APAR (Fire extinguisher Light) or with burlap soaked in water.

    3. If a fire caused by electrical short circuit turn off electricity in the building.

    4. Close the room that a fire so as not to spread to other rooms but not locked, to make it easy if it will extinguish the blaze.

    5. If a large fire, get out the room and invite all peoples around you to leave the room immediately. Do not busy yourself to collect items in the lab.

    6. Walk calmly, do not run, full attention and do not panic.

    7. If you find heavy smoke on the street or a room, avoid not to inhale the smoke, for example by way of crawl and breathing with the face close to the floor, use a damp cloth as covering the nose, this will help to breathe.

    Preparation:


    To be able to operate an LCD projector, there are some systems that need to be prepared:


    1. LCD projector.
    2. Power cable.
    3. VGA cable.
    4. Sources Voltage 110-220 Volt.

    Work Steps:

    1. Connect the projector with the power supply voltage, if the power indicator light turns Orange color means the LCD is ready to use.
    2. Open the lens cover.
    3. Press the power button about 3 minutes until the indicator light turn green and display a full featured se4lama 10-30 seconds.
    4. Turn on all equipment to be with LCD input such as: CPU, Laptop, Video Player, etc..
    5. Attach the VGA cable connector on the VGA port yng exist on your PC or laptop, then connect the VGA connector kaberl other yng on the LCD port.

    8. Ventilation holes do not be hindered by a foreign object because it would interfere with the cooling process that will shorten the time live LCD.
    9. Do not cover the lens with combustible material.
    10. Do not store liquids near Projecto.

    Maintenance LCD Projector

    1. Clean the lens using the Lens Cleaning Paper.
    2. Clean the LCD body with a soft cloth.
    3. Save the LCD on the dry reply.
    4. LCD should bring with her purse.
    5. Observe the indicator lights on the Settings> Information> Lamp Hours Time to prepare for lamp replacement.

    Nowadays, LCD is very important in education, especially in teaching learning process.
    as a teacher, lcd easier for them to teach in front of the class. teachers simply read and explain the material to their students. as a student, the lcd is also easier for them to understand the material. they can see, read and write the material.

    Here are the step to consider when using LCD display projectors:

    1. Read the LCD projector manual on its proper operation Not all LCD projectors work the same and each have their own unique operating requirements. Become very familiar with the projector BEFORE using it during your actual presentation. Make sure your computer can be properly interfaced with the LCD projector.

    2. Practice setting the equipment up several times Spend some time making sure you know how to properly set up the LCD projector with your computer and other computers. Set up the LCD projector in the actual presentation environment you will be using, if possible.

    3. Set up well in advance
    Allow yourself plenty of time to set up your computer and the LCD projector. Check any last minute details.

    4. Check the LCD projector bulb life
    LCD projector bulbs do have limited life. Some bulbs have shorter lives than others. Check and make sure the bulb you will be using is not close to the end of its life.

    5. Bring a spare bulb and cables!
    Always carry spare bulbs with you and make sure you know how to properly change the bulb. Also, remember, "Hot" glass looks like "Cold" glass; be careful and bring a towel or glove to use when changing the bulb. Practice changing the bulb during one of your practice sessions.

    6. Check your presentation color combinations
    Take some time to check out the actual presentation for the color combinations you will be using. Some colors and color combinations do not project well.

    7. Check the font size you are using
    Nothing is more frustrating to an audience than text that cannot be easily seen or read. Make sure you are using the proper text size for the distance you will be projecting your slides.

    Existing activities within the scope of the management of the laboratory includes lab work, the use of laboratory equipment, laboratory use for research and research cooperation or the like.

    f. Tidak di ijinkan memasukkan flashdisk ke computer laboratorium kecuali atas ijin petugas.

    Introduction
    An important first step in planning laboratory experiments is a comprehensive evaluation
    of the potential hazards associated with the work. This includes hazards that may be
    posed by chemicals, biological and radioactive materials, equipment, physical hazards
    and conditions. This risk analysis should not be divorced from the other components of
    the planning process, but rather integrated into a single process evaluation. This
    evaluation should be documented and used a tool for future reference in the form of a
    Standard Operating Procedure. In most cases SOPs should be developed in a
    collaborative environment, with the involvement of multiple researchers as opposed to
    individually.
    Elements of a Standard Operating Procedure (SOP)
    There are no absolute correct or incorrect methods for developing an SOP, nor is there a
    single acceptable format. The process is not intended to be about filling out forms; the
    process, when completed appropriately, ensures that safe work practices have been
    developed for the experimental work. SOPs can be integrated directly into laboratory
    manuals or procedural experimental guides, developed for a specific hazard such as an
    individual chemical or class of chemicals or specific equipment. SOPs can be stand alone
    documents or supplemental information included as part of research notebooks,
    experiment documentation or research proposals. SOPs such contain, at a minimum, the
    following information:
    • Identification: Identify, specifically, the intended scope of the SOP. The SOP
    can focus on specific processes and procedures, individual hazardous
    materials or groups of hazardous materials, equipment and conditions.
    Circumstances that would require Prior Approval, as discussed in Section 4.5
    of the LCHP should also be included.
    • Process Hazards Assessment: A hazards assessment which details potential
    hazards associated with the activity/process. This would include hazardous
    materials, physical hazards, equipment items electrical hazards, lasers etc.
    • Exposure Controls Review: The hazards assessment information should be
    utilized to consider potential exposures using the four “routes of exposure” as
    a foundation.
    • Engineering Controls: A detailed description of the engineering control
    requirements of the process/procedure along with a method of ensuring proper
    operation and efficacy.
    • Personal Protective Equipment: A detailed description of the Personal
    Protective Equipment requirements of the process/procedure along with the
    location of supporting reference information.
    • Storage and Handling Requirements: Detail storage requirements for
    hazardous materials and process handling issues specific to the procedure.
    • Proficiency and Authorization: The training requirements for the specific
    procedure, or piece of equipment should be clearly defined, along with a
    mechanism to ensure proficiency within the laboratory. Specific procedures
    for access control and authorization during the procedure should also be
    outlined.
    • Waste Management: Outline waste materials to be generated and appropriate
    waste management procedures for the work.
    • Decontamination Procedures: Specific methods for
    decontamination/cleaning appropriate to the work being completed should be
    included.
    • Emergency Procedures: Detail the specific procedures to be followed if
    emergency situations arise from the process/procedure. These steps will be
    based on the hazard assessment and controls sections.
    The Office of Environmental Health and Safety can assist in the development of
    laboratory specific SOPs. An SOP template and an example of an experimental review

    2. Connect equipment up as follows:
    * Fly Lead from TV Aerial Socket to Satellite Decoder 'TV' Socket
    * TV Aerial into Satellite Decoder 'Ant' Socket
    * One end of coax into Satellite Dish LNB socket
    * Other end of coax into Decoder 'LNB' socket

    3. Tune in equipment as follows:
    * Connect Mains
    * Switch on Mains Power
    * Switch on TV
    * Switch on Decoder
    * Tune spare TV channel to RF output of decoder (You may have to access box menus to see RF output)
    * Check Decoder setting matches LNB GHz setting (If not, make necessary alterations)
    * Set Decoder to desired channel (You may need to check frequency setting to make sure channel and MHz are correct e.g. ARD1 = 11494 MHz)
    * Move dish slowly back + forth and up + down until you see a TV picture

    VI. AC OPERATION
    - Press the button “ON” to turn on the AC
    - Press the up arrow button to warmer the temperature, and down button to colder.
    - Press the button “OFF” to turn off the AC

    4.Setelah form ”Log on to windows” tampil, maka:
    *Untuk login ke domain menggunakan user name pribadi :
    a.Isi kolom User name dengan User name Anda
    b.Isi kolom Password dengan Password Anda
    c.Pada kolom Log on to, pilih SI
    *Untuk login ke domain menggunakan user name tamu :

    For daily cleaning:
    1. Remove all debris and physical matter using a squeegee.
    2. Wipe with a clean cloth dipped in a mild soapy detergent.
    3. Mist lightly with a 200 ppm quaternary ammonium compounds (QUATS) solution.
    4. Let air dry.
    5. Inspect machine to make sure it is clean.

    For weekly breakdown:
    1 . Call maintenance to lock out all electrical sources and remove all access panels.
    2. Remove all particulate matter from inside and outside panels and all exposed machine parts.
    3. Spray with alkaline soapy detergent solution. Let it work for 15 minutes.
    4. Rinse with clean water.
    5. Let air dry.
    6. Mist lightly with a 200 ppm QUATS solution.
    7. Let air dry.
    8. Inspect machine to make sure all parts are clean.
    9. Call maintenance to put machine back together.
    Corrective Action:
    If particulate matter is found on any area of the machine upon inspection, repeat clean-up procedure as detailed above for that particular area.

    If machine fails to start after weekly breakdown, call maintenance. Do not attempt to fix machine!

    Cara Membuka Rekening BCA dengan cepat
    Untuk membuka rekening baru di BCA, yang Anda perlukan adalah:
     Foto Copy KTP.
     siapkan juga dana sebesar Rp. 500.000,- untuk pembukaan rekening awal.
    Setelah menyerahkan formulir pembukaan rekening, Customer Service akan menanyakan ke Anda, apakah Anda ingin nama Anda tertera di kartu ATM BCA yang akan dibuat. Ada dua jenis kartu ATM yaitu silver dan gold, saya menganjurkan untuk membuat yang silver saja, karena potongan administrasi perbulannya sedikit lebih murah dibandingkan dengan yang gold.
    Jika Anda ingin nama Anda tertera dikartu tersebut maka proses pembuatan kartu bisa menghabiskan waktu 1 sampai 2 minggu. Agar bisa cepat, maka sebaiknya Anda tidak perlu membubuhkan nama dalam kartu tersebut. Mintalah kartu ATM BCA tanpa nama saja. Dengan kartu ATM BCA tanpa nama, proses pembuatannya menjadi sangat cepat. Bisa 1 jam atau kurang.Setelah selesai, kartu ATM Anda baru akan bisa digunakan besok harinya.
    Biaya aktifasi KlikBCA adalah Rp. 10.000,- Jadi, setelah account KlikBCA Anda aktifkan, maksimal Anda bisa mengambil uang Anda sebesar Rp. 480.000.- Jadi, jangan khawatir jika Anda tidak punya uang Rp. 500.000,- untuk buka rekening BCAbaru. Dengan 150 atau 100 ribu pun bisa. Tetapi untuk menggenapinya, Anda bisa pinjam dulu ke teman Anda. Setelah ATM aktif, Anda bisa ambil lagi dan kembalikan lagi ke teman Anda tersebut.

    1. Tes Kesehatan
    Tes kesehatan dimulai dengan membeli formulir tes kesehatan dan pemeriksaan KTP. Biasanya yang dites hanya mata saja untuk mengetahui mata kita rabun minus atau tidak. Apabila lulus anda dapat melangkah ke babak selanjutnya, namun jika gagal maka anda tidak dapat mengikuti tahapan berikutnya.
    2. Membeli dan Mengisi Formulir Pendaftaran
    Harga formulir pendaftaran berbeda antara SIM baru dan perpanjang SIM. Untuk SIM baru biasanya berharga lebih mahal. Sebelum membeli formulir anda diharuskan membeli asuransi. Setelah diisi dengan baik dan benar anda harus menyerahkan berkas yang sudah lengkap tadi ke loket pendaftaran.
    3. Ujian Teori Mengemudi Kendaraan Bermotor
    Yang wajib anda persiapkan di sini adalah alat tulis minimal bolpen dan pensil 2B untuk komputer. Alat tulis penunjang tambahan seperti penghapus pensil, tipex, rautan dan lain sebagainya boleh anda bawa untuk jaga-jaga. Materi soal yang diujikan pada ujian SIM biasanya adalah mengenai rambu-rambu lalu-lintas, prioritas jalan di persimpangan, pengetahuan dasar kendaraan, dan lain sebagainya. Setelah ujian selesai tunggu hasil ujian dibagikan. Apabila lulus maka anda dapat melanjutkan ke ujian praktek, sedangkan bagi yang tidak lulus dapat mengulang di hari lain sesuai jadwal.
    4. Ujian Praktek Mengemudi Kendaraan Bermotor
    Ujian praktek adalah ujian membawa kendaraan bermotor yang akan anda buat izinnya. Tingkat kesulitan biasanya disesuaikan dengan mood petugas yang memberi tes anda. Jika anda lulus tes lapangan maka anda berhak mendapat SIM / Surat Izin Mengemudi dari Kepolisian.
    5. Membuat dan Mengambil Kartu SIM / Surat Izin Mengemudi
    Setelah lulus semua ujian maka anda harus membuat foto, tanda tangan, dan sidik jari digital. Semua hal tersebut dilakukan di tempat anda membuat SIM secara langsung. Jangan lupa ngaca dulu agar foto anda tidak berantakan rambutnya. Setelah membuat data djital maka anda tinggal menunggu sim diprint dan di bagikan.

    TATA TERTIB LABORATORIUM BAHASA SMA NEGERI 2 CIANJUR TAHUN PELAJARAN 2009-2010

    Demi menjaga keamanan dan kenyamanan belajar di laboratorium Bahasa SMANDA, peserta didik wajib mengikuti tata tertib yang diberlakukan seperti di bawah ini:
    1. Sebelum masuk ke ruang laboratorium Bahasa, peserta didik diwajibkan membuka alas kaki.
    1. Sebelum guru masuk ke laboratorium bahasa, peserta didik dilarang mendahului masuk.
    1. Sebelum mengambil tempat duduk, tas dan sejenisnya di simpan di depan dekat dashboard.
    1. Siswa mengambil tempat duduk sesuai dengan nomor absen atau nomor panggilan.
    1. Sebelum memakai headset, pastikan bahwa volume microphone dapat disesuaikan. Headset digunakan untuk kegiatan listening, dan microphone dapat digunakan untuk speaking ketika lampu merah pada booth menyala.
    1. Setelah menggunakan headset, posisi microphone dikembalikan ke tempat semula
    1. Tidak dibenarkan menggunakan computer atau alat elektronik lainnya, tanpa izin dan bimbingan coordinator atau guru pembimbing.
    1. Apabila terjadi kerusakan software/hardware atau hilangnya instrument pada booth setelah digunakan oleh peserta didik, maka pesera didik yang bersangkutan harus mengganti instrument yang rusak atau hilang tersebut
    1. Peserta didik dilarang membawa makanan atau minuman yang mengakibatkan timbulnya sampah.
    1. Peserta didik tidak diperkenankan menggangu/ interrupt peserta didik yang lain
    1. Peserta didik tidak diperkenankan melakukan corat-coret pada bagian manapun dari laboratorium

    1. Memberikan konfirmasi awal kepada Koordinator tentang penggunaan Laboratorium.
    1. Mengisi jurnal kegiatan penggunaan Lab yang telah disediakan setiap kali Lab digunakan
    1. Sebelum memulai kegiatan, memastikan ruangan bersih, listrik menyala, dan alat/instrument yang akan diperlukan telah siap .
    1. Pastikan CD/ kaset / DVD yang akan digunakan berfungsi dan berada pada posisi point menit yang akan digunakan
    1. Memastikan peserta didik telah menempatkan headset, microphone dan kursi pada tempat semula
    1. Mematikan listrik sebelum meninggalkan ruangan dan mengunci ruangan.

    How To Operate a Laptop Computer

    Level of difficulty: Easy

    The proper operation of a laptop computer ensures that the laptop not only works properly but also protects the user from any injury or harm. Improper ways of operating a laptop computer and not following the prescribed operating procedures and safety issues can lead to irreparable damage to the laptop computer. An improperly operated laptop can also cause injury to the user such as a laptop burn. A laptop computer has different operational needs as compared with a desktop computer hence should be handled and managed in a more delicate manner as that of desktop computers.

    Materials Needed:
    - Laptop Computer
    - Internet
    Step 1
    The laptop computer has to be shut down whenever it is not in use to prevent it from overheating. Every laptop user must remember that laptop computers generally warm up faster than desktop computers. To help the laptop computer avoid overheating when not in use even for short durations, the user may make adjustments on the power options provided in the laptop computer. The user can program the hard drive and display to turn off after preset time duration. Another option for the user is to set the computer into hibernate or standby mode.
    Step 2
    The user should shut down the computer before placing the laptop in its carrying bag since the laptop can melt when left on, particularly when the notebook is placed in a container where no air or limited air is allowed to circulate.
    Step 3
    The laptop’s air vents must also be periodically inspected and cleaned to keep these air vents free from debris. However, the user must not force anything into the air vents to clean its inside as this may cause damage to the laptop computer. The laptop fan should also be checked since an improperly working laptop fan can be a cause for overheating. To test the laptop fan, the user may download the software from the laptop manufacturer’s website or online support. In some cases where the laptop fans are controlled through BIOS, the user may check online from the manufacturer’s website for any BIOS updates.
    Step 4
    The user must also ensure that the laptop is operated on a hard surface and preferably one that provides ventilation. Users are not advised to use a soft material between the user and the laptop since soft materials can block the flow of air and thus create overheating. A heat sink base could be used if the use of a soft surface is unavoidable.
    Step 5
    The user must also unplug any accessories whenever the laptop computer is not in use. These accessories not only consume power but they could also lead to overheating of the laptop.

    SOP Cara membuat Kartu Keluarga ( KK ):

    Kartu Keluarga yang selanjutnya disingkat KK adalah Kartu yang Kemuat Data Kepala Keluarga dan semua anggota keluarga.

    Persyaratan :
    1. Surat Pengantar dari RT/RW yang menyatakan bahwa penduduk yang bersangkutan adalah penduduk setempat, tetapi belum memiliki KK.
    2. Kartu Keluarga lama ( bagi penduduk yang telah mempunyai KK lama, tetapi ada perubahan data Kepala Keluarga dan atau anggota keluarga).
    3. Surat Ijin Menetap (SIM) bagi penduduk pendatang.
    4. Akta Perkawinan/Akta Nikah/Akta Perceraian.
    5. Akta Kelahiran/Akta Kematian.
    6. Surat Keterangan Ganti Nama (bagi yang telah berganti nama).
    7. Surat Bukti Kewarganegaraan Republik Indonesia .
    8. Surat Bukti Penyerahan Data Kependudukan;
    Untuk WNA, harus melampirkan :
    KITAP / SKK A/B dari Kantor Imigrasi.
    9. Surat Bukti Penyerahan Data Kependudukan WNA;
    Buku Pengawasan Orang Asing (BPOA) dari Kantor Imigrasi;
    10. Surat Keterangan Lapor Diri (SKLD) dari Kepolisian.

    Prosedur :
    1. Pemohon mengisi Formulir master KK yang berisi data – data Kepala Keluarga dan anggota keluarganya yang ditandatangani oleh Kepala Keluarga, RT/RW dan Lurah.
    2. Membawa Formulir Master KK ke Kelurahan dengan melampirkan persyaratan sebagaimana tersebut di atas.
    3. Kelurahan menerima dan meneliti berkas permohonan beserta persyaratannya, memberikan nomor register dan mengirimkan berkas beserta persyaratannya ke Kecamatan.
    4. Kecamatan menerima dan meneliti berkas permohonan beserta persyaratannya, kemudian melakukan proses input data berdasarkan data-data yang ada dalam Master KK.
    5. Kartu Keluarga yang telah diproses, diteliti kembali dan apabila sesuai dengan data -data yang ada, maka Camat menanda tangani Kartu Keluarga dalam rangkap 4 (empat).
    6. Kecamatan mengirimkan Kartu keluarga lembar 1, 2 dan 3 ke Kelurahan, sedangkan lembar ke 4 diarsipkan di Kecamatan.
    7. Kelurahan menyerahkan Kartu Keluarga lembar ke 1 kepada pemohon/kepala keluarga dan lembar ke 2 kepada pengurus RT, sedangkan lembar ke 3 diarsipkan di Kelurahan dan mencatat data penduduk berdasarkan Kartu Keluarga dalam Buku Induk Penduduk.

    Masa Berlaku Kartu Keluarga ( KK ):
    Kartu Keluarga berlaku selama tidak ada perubahan data (mutasi data) dari Kepala Keluarga dan anggota keluarganya.

    Penggantian Kartu Keluarga (KK) dilakukan apabila :
    1. Terjadi penambahan/pengurangan anggota keluarga, perubahan data, perubahan status dan lain-lain yang merubah data Kependudukan (mutasi data) Kepala Keluarga dan anggota keluarganya.
    2. Kartu Keluarga rusak.

    7. Prepare the material in slide of presentation or by words in white board if necessary.

    9. Switch off the computer when you have finished of using it.
    10. Check your bag, book, or the other thing. Don’t forget to bring it completely.

    Rescue:
    - Don’t bring litter.
    - Don’t bring food or drink.
    - Switch of the electricity.
    - If you find any fire, splash emergency jar on it.

    Pencegahan dan Penanganan Virus

    a. Pengguna diharuskan untuk memeriksa dan membersihkan virus dari setiap flasdisk yang akan digunakan.
    b. Pengguna diharapkan untuk selalu mengupdate antivirus yang sudah terpasang.
    c. Pengguna dilarang keras mengubah konfigurasi peralatan (mengubah, menambah maupun mengurangi perangkat lunak maupun perangkat keras) yang berada di laboratorium komputasi.
    d. Pengguna dilarang meninggalkan computer dalam keadaan on.
    e. pengguna wajib menjaga kebersihan Lab Komputer.
    Sanksi :
    Apabila terjadi penyimpangan terhadap peraturan diatas, maka pengguna akan dikenai sanksi sesuai dengan ketentuan yang berlaku di Lab Universitas Muhammadiyah Surakarta.

    SOP Cara Menghubungkan Tampilan Laptop ke LCD Proyektor.

    Ada beberapa setting untuk menampilkan tampilan laptop ke LCD Proyektor, yang pastinya anda harus menghubungkan Laptop anda dengan LCD Proyektor dengan DATA Monitor. Setelah itu ada beberapa jenis Laptop yang langsung mendeteksi dan menampilkan melalui LCD Proyektor. Tapi ada juga yang harus harus di setting terlebih dahulu. Untuk jenis Settingannya adalah sebagai berikut :

    Untuk Lapotp Compaq : Tekan fn+F4 (tunggu beberapa detik karena LCD proyektor akan mencari perangkat). Ada beberapa mode jika fn+F4 (F4 ditekan 1x) biasanya tampilan hanya di LCD Proyektor sedangkan di Lapotop akan Blank, jika fn+F4 (F4 di tekan 2x) maka akan muncul di Laptop dan LCD Proyektor. Nah ini cara yang pertama, cara kedua adalah…..

    Setting Dekstop

    1. Masuk ke dekstop Laptop Anda
    2. Klik kanan di Dekstop
